Insurance Information

Dental insurance is diverse. Some policies cover only a small percentage of fees, while others cover 50 percent or greater. The more you or your employer pays for premiums, the better the coverage, in most cases. Dental insurance can be confusing, but we'll make it easy for you. Simply bring your card or claim into our office. We'll be happy to tell you what your insurance will cover and file your claim for you.

We Offer Our In-office Dental Health Plan For Patients Who Do Not Have Dental Insurance

Benefits of our dental plan include:

• No annual maximum
• No deductible
• No wondering whether insurance will pay towards your treatment

• No preauthorization is ever required!

Membership Benefits:

• Initial comprehensive exam, X-rays, and Healthy Mouth Cleaning
• Six-month checkup with exam and Healthy Mouth Cleaning
• One emergency exam and X-ray
• All dental treatment completed within 12 months of enrollment will be discounted by 20% if paid in full at time of service. This includes elective cosmetic procedures like teeth whitening, crowns, and veneers.

Annual Cost for Individual Plan: $299

Annual Cost for each additional family member under the age of 14: $199

There's no ID card, no group nor member number to bring! All your membership information will be kept in your electronic record. Your effective date is the day you sign up and your renewal date is the same date every year.

"The Fine Print" This is a dental discount plan and is NOT dental insurance. It CANNOT be combined with any other dental insurance. It is good ONLY at Pro-Dental Care. Therefore, if you are referred to a specialist, they will NOT offer this discount. Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, car, disability or workman's comp type insurances are involved, this discounted plan cannot be used.

Non-refundable. No refunds given if the patient chooses not to use their dental plan. The plan is subject to change yearly. All payments are due AT TIME OF SERVICE to receive the discount. Any services received that are not paid for at the time of service will be billed at the usual, higher fee. If you choose to extend your payment for treatment by paying through CareCredit, the restorative treatment discount is reduced to 10% due to merchant fees. This offer cannot be combined with any other offers. Plan fees are valid only when paid at time of enrollment.
